Offer description

A Specialist Community Public Health Nurse in Health Visiting to provide strong clinical care and leadership within an integrated skill mixed team as part of the 0-19 Universal Health Service in Blackburn with Darwen.

To lead on the delivery of the Health Child Programme 0-5 years and defined local requirements based on the provider contract.

To provide family centred, evidence based public health service that aims to proactively improve health outcomes for a defined caseload, population or community, reducing the risks and impact of health inequalities by working in partnership with families, communities and other professionals. This will be achieved through delivery of the model of health visiting at a community, universal, targeted and specialist level. Safeguarding is embedded within the model considering safe and effective management of risk for children, young people and families.

Deliver the Healthy Child Programme (0–5 years) – offering developmental reviews, health advice, and early interventions.

Support parents and carers – providing guidance on feeding, sleep, child development, and emotional wellbeing.

Identify needs early – spotting potential health, developmental, or safeguarding concerns and arranging appropriate support.

Promote health and wellbeing – encouraging healthy lifestyles, immunisations, and positive parenting.

Work in partnership– collaborating with GPs, midwives, social workers, and other community services.

About You

We're looking for someone who is:

• A Specialist Community Public Health Nurse (SCPHN)-Health Visitor.

• Experienced in supporting families with complex health and social care needs.

• Skilled at building strong relationships with families and professionals alike.

• Passionate about reducing inequalities and improving health outcomes for children.

• Confident in safeguarding practice and risk management.

• Skilled in using a range of assessment tools to assess physical and emotional needs and parental motivation to change

• Keen to learn and develop for self-improvement as well as for the benefits of families in the 0-19 Service

• Motivated, solution focussed, compassionate, and resilient in the face of challenges.
