We have an exciting opportunity for a Long Term Nurse to join our Primary Care team at HMP Wormwood Scrubs, a category B Prison located in Hammersmith and Fulham in West London.

In this role you'll be working with a multi-disciplinary team that will support you and help you succeed.

Hours

Monday to Friday from 07:30 - 16:30

Salary

This position is full time, you'll receive an annual salary up to £53,172 per annum - depending on experience.

• Our bespoke Learning Management System to address your learning needs
• Support from the wider team

Candidates are required to have a Legal right to work in the UK and due to the nature of the role, offers of employment will be subject to additional vetting and security checks.

What you'll be doing

As a Long Term Conditions Nurse, your responsibilities will vary. You will

* You will identify immediate health needs including, any external services or prescribing requirements that prisoners
may have received e.g. prior to remand or repatriation onto a wing
* A key part of this role will include the facilitation of arrangements between agencies involved in delivering a complex care package to ensure effective collaboration and stability for the patient in delivery of that care. This requires close joint working with other in-house healthcare services and departments within the prison such as Pharmacy, Education, Offender Management, Substance Misuse and Physical Education, etc.
* Deliver continuity of care across health and social care agencies from courts, to custody and community whilst improving the communication between the above.
* Inspire the continuous improvement of MDT working across HMP and healthcare teams.

Requirements
* NMC Registered Adult Nurse
* Knowledge of health and social care models across the criminal justice system
* Knowledge of Evidence Based Practice and Clinical Governance
* Experience of assessing the needs of and working with individuals with complex health and social care needs.
Although not essential ideal applicants would have experience in a hospital, community or prison environment.
