Are you passionate about making a difference in the lives of young adults navigating mental health challenges? A new exciting opportunity has arisen for a Band 7 Young Adults Pathway practitioner to join the Bentley House Community Health Hub.
We are looking for a qualified mental health clinician to work with the Harrow Young Adult Pathway Lead and provide a high quality, accessible mental health pathway for 16‑25 year olds ("young adults") in the borough. The pathway aligns with the Trust's model of care for Young Adults and looks to provide a high quality, safe and effective pathway tailored for the 16‑25 age range, aiming to reduce gaps, address unmet needs, facilitate smooth transitions, and ensure a positive service user experience.
You will need to be a dynamic and flexible team player with excellent communication skills. Previous work experience in supporting individuals with mental health and neurodiverse needs would be desirable. The service is a short walk from Harrow & Wealdstone station with good transport links.
Main duties of the job
* To work with the Young Adults Pathway Lead to identify and support young people with mental illness and at risk mental state patients across the whole system.
* To focus on health inequalities for young people between 18‑25 to deliver a holistic care to young people using a bio‑psycho‑social approach which is trauma informed.
* Maintaining and developing the good relationship between EIS Harrow, Harrow Whole systems and young adults partnership model of care team (Harrow YAP) in delivering a service to young adults at risk of serious mental illness.
* To demonstrate excellent engagement skills, an anti‑oppressive and enabling approach to co‑production with the young people patients, and to be skilled in assessing mental health, creating risk assessments, safety plans using DIALOG Plus and crisis and contingency plans.

The Bentley House Community Mental Health Hub operates an intervention‑based model and utilises a trauma‑informed approach to working with our service users. Regular supervision and access to opportunities for professional development will be offered to the successful postholder.
Job responsibilities
* Act as a core member of YAP team with active presence in YAP Forum meetings.
* Carry a small case load of complex and challenging patients with mental illnesses with comorbid neurodiversity in Adult CMHH.
* Provide day‑to‑day management to support in delivering community care to service users and their carers based on the intervention‑based model of care.
* Work closely with the YAP Lead, Team Manager and other senior staff.
* Establish strong working relationships with the in‑patient Service, Harrow Crisis Resolution Team, Harrow Psychiatric Liaison Team, Early Intervention Service and CNWL Single Point of Access.
* Attend the acute services bed management meeting and Lavender walk bed management meeting, once a week to update on CMHH/YAP Lead on actions and feedback to CMHH and vice versa to facilitate any further actions on transfer of care.
* Work closely with YAP lead to identify the CAMHS cases transitioning to AMHS, sign post in the third sector, help them engage with partner organisations.
* Facilitate in‑patients and HTT transfers in a timely way for complex young people when requested.
* Offer regular structured supervision to staff within the team, as well as providing informal support and advice to them and to any other team members in the course of their work.
* Undertake regular caseload scrutiny for categories such as discharge, step‑down or step‑up care.
* Ensure that required documentation, activity reports and quality standards are maintained within the Teams.
* Ensure that Trust policies are implemented, including clinical policies, Health and Safety, Fire, Sickness, the reporting of Untoward Incidents and Accidents, and to ensure that the Team Manager is informed of any such incident or any complaint. Assist in the investigation of such matters, at the Service Manager's request.
* Participate fully in HUB MDTs to facilitate care planning for young people where required.
* Facilitate transfers from Acute in‑patients team to EIS and from EIS to CMHH Hub in a timely way for young people between ages 18‑25.
* Act as Young adults Champion in the CMHH.
* Train and deliver trauma‑informed approaches and stabilisation manual to the group of young adults on the caseload.
* Take part in quality improvement projects to ensure high standards of delivery of care are achieved and maintained.
* Maintain good relationship with VCSE linked with YAP and Peer Support Team.
* Have an active role with other senior colleagues in the continuing development of a high quality service that is underpinned by evidence‑based practice, care pathways and NICE guidance.
* Possess good communication skills and be able to form positive, therapeutic relationship with service users and carers and staff.
* Communicate sensitively both verbally and non‑verbally with service users and maintain good relationships with carers and others.
* Contribute to Quality Improvement projects.
* Deliver training specific to the role to members of the MDT.
* Act as a role model to colleagues, always seeking to maintain the highest standards of professionalism.
* In delivering your duties you are expected to display behaviours that are aligned with the trusts core values.

* Maintain contemporaneous records of sessions and clinical activity on S1.
* Participate in yearly CNWL appraisal and continue to develop personal skills by participating in continuous professional development.

* Support the involvement of users and carers in discussions about their treatment and care as appropriate and help their voice to be heard.
* Undertake regular audit of client case notes to ensure high standards of delivery of care are achieved and maintained.
Person Specification
Skills and Knowledge
* Knowledge of NICE guidelines relevant to ARMS, Early Intervention and Psychosis.
* Highly developed specialist knowledge, both theoretical and practical, of CBT applied to clinical practice, particularly in respect of adults and young people with severe and enduring mental health disorders (including personality disorders).
* Ability to manage emotionally stressful situations such as working with victims of abuse or trauma, or with people who engage in severe self‑harm or aggressive behaviour.
* Risk identification and management in ARMS and the early course of psychosis.
* Competency in CAARMS‑2023.
Qualifications
* Undergraduate degree in relevant area (e.g. psychology, social work, occupational therapy, mental health nursing, youth work) (A)
* Full registration with relevant professional regulatory body (e.g. HCPC, BABCP, AFT, UKCP, NMC, Social Work England, National Youth Agency) (A)
* NMP/Diploma/higher education in quality improvement.
Experience
* Experience of delivering CBTp to NHS clients.
* Experience working in an NHS Multidisciplinary team.
* Experience of working in a multi‑cultural setting, including working with interpreters.
* Experience of an NHS ARMS service.
* Experience of delivering CBTp in an Early Intervention in Psychosis Team.
* Experience of being a Care Co‑ordinator for severe mental illness patients.
