Main duties of the job

Deliver safe, effective, and holistic patient care in accordance with current clinical guidelines.

Manage a wide range of patients, including those with acute and long-term conditions.

Conduct home visits and provide urgent care when required.

Work collaboratively with the wider practice team, including nurses, healthcare assistants, and administrative staff.

Contribute to clinical governance activities, audits, and ongoing service improvement initiatives.

Ensure accurate, comprehensive, and timely completion of all medical records and documentation.

Actively participate in team meetings and practice-wide activities to support a cohesive, patient-centred service.


About us

We are a supportive and successful trainingpractice of 9,500 patients and you would be joining a robust MDT made up of 5 Partners,1 salaried GP, 1 GP Retainer, Registrars, Nursing team, Paramedic, Pharmacyteam, as well as our innovative management and admin team.

We work closely with our PCN who provide aparamedic-led home visiting service for our frail and housebound patients,meaning there are minimal home visits for our GPs, a first contact physioservice, social prescribing team, and link with mental health and wellbeingcoaches.


Job responsibilities

Deliver high-quality, holistic care to registered patients through face-to-face and remote consultations, telephone triage, home visits, and clinical administration.

Assess, diagnose, investigate, and manage a wide range of acute and long-term conditions, providing compassionate, patient-centred care.

Prescribe safely and effectively in line with current clinical guidance and best practice.

Support patients with complex or multiple health needs, ensuring continuity and coordination of care across the wider healthcare system.

Maintain accurate and timely medical records in line with GMC standards and practice policies.

Play an active role in achieving Quality and Outcomes Framework (QOF) targets, Enhanced Services, and other performance indicators.

Get involved in practice development through audits, significant event reviews, and clinical governance activities.

Work collaboratively with a friendly, multidisciplinary team, contributing to practice meetings, MDT discussions, and safeguarding work for adults and children.

Uphold high standards in infection control, information governance, confidentiality, and health and safety.

Commit to ongoing professional development, appraisal, and revalidationyour growth and learning will be fully supported.


Person Specification


Qualifications

* Fully registered with the GMC and licensed to practise
* Inclusion on the NHS England Medical Performers List
* Evidence of ongoing professional development
* Experience of managing patients in a primary care setting
* Experience of acute and chronic disease management
* Experience of preventative care and health promotion
* Understanding of primary care and the GP contract
* Awareness of safeguarding procedures for adults and children
* Knowledge of QOF, Enhanced Services, and national primary care targets
* Excellent clinical and consultation skills
* Ability to make sound clinical decisions based on evidence
* Effective communication and interpersonal skills
* Good organisational and time management skills
* Ability to work independently and within a team
* Commitment to high-quality patient care and safety
* Flexibility to meet the needs of the practice
* Willingness to engage in appraisal, CPD, and revalidation processes
* Additional postgraduate qualifications in specialist areas
* Previous salaried or partner GP role
* Experience of teaching, mentoring, or training students/trainees
* Experience of audit, clinical governance, and quality improvement work
* Knowledge of local health economy and community services
* Understanding of population health and health inequalities
* IT literacy with primary care systems (Emis Web)
* Skills or interest in service development or leadership
* Interest in developing a specialist clinical area
* Willingness to take on additional responsibilities within the practice


Experience

* Ability to work under pressure and prioritise workload
* Proficient in the use of clinical IT system
* Excellent interpersonal skills and communication skills
* Ability to work as part of a multi-disciplinary team
* Ability to work with limited supervision
* Understanding of the pressures faced by GPs and healthcare teams
* Appropriate representation with a recognised medical defence organisation
* Current DBS check (undertaken less than 6 months before the date of appointment)
* Proven ability to offer support within a clinical team and share learning points in Practice meetings
* Proficient in the use of EMIS Web / Docman10
* Experience of working in a GP practice
