As a GP Practice Pharmacist, you will work as part of a multidisciplinary team within a GP practice to improve medicines optimisation, patient outcomes, and overall service delivery. Providing face to face and remote consultations, advice, treatment and administrative support alongside a wider team of healthcare professionals.


Main duties of the job

You will provide clinical medical reviews whilst supporting and advising patients with chronic disease management issues. This includes helping to manage long-term conditions, advising those taking multiple medicines (polypharmacy) and delivering clinical advice about treatments.

We are a large innovative GP Practice based over three sites, offering a comprehensive range of patient‑centred services for the areas of Central and West Worthing, Tarring, Goring and Durrington.

Worthing Medical Group is the largest GP surgery on the South Coast with a patient list of nearly 21,000.


Key responsibilities

* Providing clinical expertise and being an educational resource for colleagues in relation to medicines (e.g. doses, side‑effects, alternatives) and treatment pathways whilst helping to address the public health and social needs of patients within NICE and other evidence‑based guidelines
* Conduct medication reviews, including for patients with long‑term conditions, polypharmacy, frailty, and high‑risk medicines within a defined scope of practice
* Work collaboratively with the Senior Clinical Pharmacist as a part of the Clinical Pharmacy team to ensure continued delivery of high‑quality, safe, and effective medicines optimisation
* Supporting the medicines management team to help with handling patients and healthcare professionals prescription queries and providing remote support where needed
* Supporting with the managing, auditing and reviewing prescription and repeat prescription policies to improve the quality, safety and cost‑effectiveness of prescribing
* Ensure effective and appropriate monitoring and reviews are completed for high risk medications
* Support the process of medicines reconciliation on admission, following discharge from hospital and clinic letters
* Identify and address medicines optimisation and safety issues, including deprescribing where appropriate
* Support the implementation of medicines safety alerts and national guidance
* Participate in clinical audits, quality improvement projects, and prescribing reviews including QOF and MOIS
* Liaise with and proactively develop relationships with a multidisciplinary team (including colleagues, NHS professionals and private organisations) and act as a contact for medicines advice for the practice
* Registered with the General Pharmaceutical Council (GPhC)
* Minimum 2 years experience working in patient‑facing pharmacy roles (e.g. community, hospital, or primary care)
* Good clinical knowledge of medicines used in primary care
* Strong communication and interpersonal skills
* Ability to work independently while recognising own limitations
* Commitment to ongoing professional development
* Independent Prescriber qualification (or working towards)
* Experience working in general practice or primary care networks
* Completion of, or working towards, Primary Care Pharmacy Education Pathway (PCPEP)
* Experience conducting structured medication reviews
* Knowledge of EMIS clinical system


Experience

* Minimum 2 years experience working in patient‑facing pharmacy roles e.g. community, hospital, or primary care
* Good clinical knowledge of medicines used in primary care
* Strong communication and interpersonal skills
* Ability to work independently while recognising own limitations
* Commitment to ongoing professional development
* Experience working in general practice or primary care networks
* Completion of, or working towards, Primary Care Pharmacy Education Pathway PCPEP
* Experience conducting structured medication reviews
* Knowledge of EMIS clinical system


Qualifications

* MPharm degree or equivalent
* Registered with the General Pharmaceutical Council GPhC
* Independent Prescriber qualification or working towards
