To work as part of the multi-disciplinary team to provide an effective and efficient response to referrals from the Emergency Department and Medical wards in the Acute trust. To assess, risk manage, and appropriately refer and signpost all patients referred who need assessment and/or follow-up from mental health services. As part of the team, to ensure the development of effective interfaces between the team and other mental health services, and contribute to the development of the team's gateway and assessment role. To provide information and clinical support to police and ambulance staff to help them make appropriate decisions.

Job responsibilities

If you are a registered mental health professional with experience in assessing and supporting individuals of any age, and wish to be part of a growing service, we would like to hear from you. You will undertake comprehensive mental health and risk assessments, develop risk management plans, provide brief interventions, and make appropriate onward referrals within hospital services. The role offers diverse opportunities to develop skills across service areas, emphasizing holistic, person-centered care and collaboration with non-mental health colleagues.

You will also be involved in interfacing with other teams, mental health services, and agencies such as the Police, NWAS, and local authorities. The service operates 24/7, including shift work, bank holidays, weekends, and some night duties.


Main Duties and Responsibilities

* Assess patients' mental health needs and facilitate access to appropriate services.
* Manage presenting risks and liaise with families and agencies as needed.
* Ensure safe discharge of patients back to GPs when appropriate.
* Prioritize referrals and manage workload effectively.
* Provide evidence-based assessments and short-term interventions.
* Maintain accurate records and input data into information systems.
* Participate in service development, team meetings, and quality audits.
* Support police and NWAS with follow-up for frequent mental health presenters.
* Contribute to performance recording and service funding opportunities.

Person Specification


Education / Qualifications

* Professional registration in health or social care (e.g., RMN, Dip SW, BSc Occupational Therapy) with up-to-date accreditation.
* Degree-level education.
* Relevant skills-based training and management/teaching qualifications.


Knowledge

* Understanding of national legislation and guidelines.
* Knowledge of evidence-based mental health practices.
* Ability to conduct effective risk assessments and management.
* Familiarity with the Mental Health Act, Mental Capacity Act, and DOLS.


Experience

* Significant post-registration experience in acute mental health settings.
* Experience working with children, young people, and in crisis teams.
* Experience supervising staff and leading multidisciplinary teams.

* Assessment, care management, and planning skills.
* Partnership working with colleagues, service users, and carers.
* Proficiency in evidence-based practices.
* Professional judgment and strong communication skills.
* Ability to work effectively under pressure.

Work Related Circumstances

* Flexible working across divisions if required.

* Willingness to work shifts, including nights and weekends on a rotational basis.


Disclosure and Barring Service Check

This role requires a Disclosure and Barring Service (DBS) check due to regulated activities.

Salary: £37,338 to £44,962 per annum, pro-rata.
