Community Staff Nurse (Core Nursing Team)

We are for someone wanting full or part time to join our Long Eaton Core Community District Nursing Team to compliment our current staff. The post's could also involve working across other teams in the Erewash locality, including the rapid service which is also based there.

Main duties of the job
Provide high-quality nursing care to our patients in our local community to keep them safe at home, prevent hospital admission and support hospital discharge home.
Assess, plan, implement and evaluate individualised patient care in the most efficient and clinically effective way, whilst maintaining the patient's privacy and dignity.
Provide advice and support to junior staff in the delivery of high quality care and actively support the District Nurse in the management of the health care environment.
Implement / refer on to the relevant services as appropriate.
Work in close collaboration with the Integrated Community Teams/GP surgeries and local acute trusts.

Person Specification
Education / Qualifications
* NMC registered Nurse (including newly-qualified and soon-to-be-qualified)
* Evidence of recent relevant training
Experience and Knowledge Required
* Experience of working in the community
* Evidence of research based/reflective practice
* Knowledge of local and national priorities.
* Understanding of the wider NHS agenda regarding community services
* Experience of long term conditions
* Experience of Leg Ulcer management and lower limb assessments
Skills and Attributes
* Excellent communication and interpersonal skills
* IT Skills
* Written and recording skills
* Ability to supervise junior nursing staff
* Ability to assess, plan and implement care
Other
* Ability to use transport as you will be expected to travel across the Derbyshire County visiting patients' homes and across the county to access meetings and training
