Overview
Fixed term 12 Months. NHS Fife Pharmacy and Medicines Directorate seek an enthusiastic individual to join the Frailty and Hospital at Home Pharmacy Team, delivering safe, timely and efficient medicines management and supply for patients receiving care at home.
Responsibilities
* Work at ward/clinic level with the Frailty and Hospital at Home Pharmacy Team and the wider Pharmacy service.
* Enhance patient experience and facilitate communication across care providers.
* Assist in taking medication histories.
* Manage medicines supply and maintain accurate records.
* Ensure safety and effectiveness of medicines in the patient’s hospital journey.
Qualifications and Requirements
* Experience working in a pharmacy team.
* SVQ in Pharmacy Services at SCQF Level 6 (Full award) or equivalent.
* Completion of or willingness to work towards the underpinning knowledge programme for Pharmacy Support Workers at Fife College.
* Excellent interpersonal and communication skills; patient-focused with a passion for care.
* Forward-thinking, flexible, and comfortable with face-to-face interactions with patients and multi-disciplinary teams.
* Ability to travel throughout Fife.
* Become a member of the Protection of Vulnerable Groups (PVG) Scheme prior to appointment.
* Right to work in the United Kingdom and eligibility for sponsorship where applicable.

* Team works across Acute and Community hospital sites, General Practices, Mental Health services, and specialist teams.
* Fife has good transport links; distance from major cities is manageable.
