We are looking for a Social Worker to join our Follow On team, this will be primarily working closely with patients who have been discharged from hospital by our ‘Home First Hub’ but require long term care planning. Our team is experienced with working with complex cases, have a sound knowledge of the Care Act (2014), Mental Capacity Act (2005) and Safeguarding, as well as excellent time management and IT skills. Any new staff are supported to develop a thorough working knowledge of Norfolk County Council processes and procedures. Covering South Norfolk, our team of Social Workers, Assistant Practitioners and Occupational Therapists use the Care Act (2014) to assess and provide support to adults living in the community.
