Experienced social worker required to work in the Integrated Discharge Team (Contact Cares) based in Whiston Hospital. This will entail assessing patients and other tasks associated with facilitating hospital discharge, to support patients with the support they require to return home or to be discharged to another setting. The role is very fast paced and high pressured and experience in this area would be preferred. This will be a full time post (37 hours per week) between the hours of 8.45am and 5.15pm. There will likely be a mixture of working in the office and working from home on a rota basis, and will involve ward visits to assess patients where necessary.
The following is essential criteria:
– Degree in Social Work
– SWE Registration
– Right to work within the UK

You must have:
1. A professional qualification in social work
2. Social Work England registration.
3. Post qualified experience.
4. Enhanced DBS/Update service.
5. References required for the last 5 years of employment.
