Overview

As we continue to expand the Universal Crisis Team (UCT) in Northumberland and North Tyneside, we are delighted to offer opportunities for motivated and committed individuals to work as Community Practitioners within the Working Age Adult Pathway. These roles are based at St Georges Park Hospital in Morpeth and cover the localities of Northumberland and North Tyneside, with scope to work across areas with countryside and coastlines. You will be part of a supportive, friendly and welcoming Team with opportunities to grow and develop in your role and to be involved in service development and improvement.

The successful candidates will be required to work as part of the Northumberland/North Tyneside Working Age Adult Crisis Team and may be asked to work flexibly throughout the service if needed. The team prioritises staff wellbeing and flexible working. The UCT provides urgent assessment for those in mental health crisis, home-based treatment, facilitates admission to/early discharge from hospital, and supports the coordination of Section 136 referrals. The successful candidate will be expected to complete all aspects of Crisis work and to provide specialist advice regarding mental health and risk to service users, families/carers, and other professionals.


Responsibilities

* Share a caseload within and supported by a skilled MDT to deliver safe, effective, responsive and compassionate care to service users.
* Assess, formulate and manage risk and complexity within mental health/learning disability services.
* Provide care and interventions to service users with a range of complex mental health needs and associated risk.
* Develop skills by contributing to care delivery and service improvement and promoting recovery-based approaches for adults with mental health-related needs.


Qualifications and requirements

* Post is suitable for Registered Nurses, Social Workers or Occupational Therapists with current professional registration.
* Clinical experience in assessing, formulating and managing risk within mental health / learning disability services.
* Collaborative and enthusiastic approach with a passion for developing and promoting recovery-based approaches for adults with mental health-related needs.
