Main duties of the job
* Deliver high-quality, person-centred rehabilitation for adults in the community.
* Assess, plan and implement tailored treatment and rehabilitation programmes.
* Work collaboratively within an interdisciplinary team across community setting.
* Communicate effectively with patients, families and carers, supporting goal-setting.
* To manage a caseload, using evidence based/person centred principles to assess, plan, implement and evaluate therapy interventions.
* Accurately record assessments, monitor progress and adjust care plans as needed.
* Support service efficiency, audit activity and performance monitoring. Supervise and develop rehabilitation assistants as required.
* Maintain professional standards, mandatory training and uphold Trust values, safeguarding and governance.
