Role Overview:
Thirteen Group is on the lookout for a Living Well Co-ordinator to join us in our Specialist Housing team.
Our extra care and sheltered schemes offer housing for people aged 55 plus who need housing related support to live independently in their own home. This is a roam role and you’ll be based within one of sheltered schemes, but you’ll be expected to cover other schemes across the Tees Valley when required.

Key Responsibilities:
* Provide a high-quality housing management and support service that supports older and vulnerable customers to feel safe and be supported to live well and independently for as long as possible.
* Organise and complete support plans to identify risk factors, safeguarding concerns and other interventions that enable customers to live happily and independently within an extra care setting.
* Work in partnership with families, care providers, health professionals and other organisations to promote healthy lifestyles amongst customers
* Arrange social events, in collaboration with charitable organisations to create a settled, friendly and vibrant community.
* Manage schemes, ensuring buildings are managed in line with health and safety requirements, including the completion of compliance checks
About You:
* We’re looking for someone who is a skilled influencer and communicator, and you’ll be passionate about providing a high-quality service
* In-depth knowledge of safeguarding policies and procedures, the benefit system and financial inclusion. You’ll have a track record of dealing with housing management issues
* Ideally, you’ll have experience of working with or supporting older and/or vulnerable people, with an understanding of support plans and risk assessments
* With empathy at the heart of your approach, you’ll have experience of developing and managing a customer-driven service, with an ability to analyse and solve day to day problems
* You will have a full UK driving license and access to a vehicle
