What we are looking for:
* Strong written and verbal communication skills.
* A proactive, compassionate approach to patient support.

* Car driver with a valid UK driving licence and access to a car for business use.
What you will do in this role:
* Deliver neuro-rehabilitation programmes in outpatient, home, and community settings under the guidance of qualified therapists.
* Support Physiotherapy and Occupational Therapy interventions.
* Monitor patient progress and contribute to therapy planning within agreed guidelines.
* Support neuro-rehabilitation by implementing therapy programmes devised by the lead therapist.
* Manage your own patient caseload, assessing and treating patients, and monitoring their progress.
* Assist in patient assessments, gathering relevant information, and adapting treatment plans under therapist guidance.
* Work closely with the multidisciplinary team, contributing to patient care planning and discharge processes.
* Carry out basic assessments of patients’ needs as directed by Qualified Clinicians.
* Conduct mobility and transfer assessments for specific equipment as requested and report findings to the qualified member of the team.
* Identify more complex issues requiring assessment by qualified clinician.
* Implement therapy programmes (as prescribed by the OT or PT) for individuals and provide feedback to therapists.
* Carry out programmes devised by therapists using a range of techniques within competence.
* Review patient progress and provide feedback at all stages of patients progress in caseload discussions.
* Work jointly with clinicians in cases which are complex and may require physical moving and handling as part of rehabilitation.
* Be responsible for assessing and managing clinical risk in relation to own caseload, seeking guidance where appropriate.
* Will not attempt any procedure without appropriate training and competence.
* Contribute to evaluations of therapeutic interventions including planning discharge.
* Set/review SMART goals with patients, adapting them as patient progresses.
* Monitor progress with appropriate outcome measures, adjusting treatment as appropriate.
* Prioritise workload and plan day-to-day workload.
* Identify factors involved in making decisions with regard to prioritisation.
* Prioritise referrals to rehabilitation assistants on a daily basis.
* Provide support to clients, their carers, and other professionals involved in a patient's care journey.
* Use listening skills and be aware of issues and how to respond to questions and anxieties, demonstrating empathy, sensitivity and good interpersonal skills.
* Liaise with family, carers or others involved in a patient's care to teach exercise programmes for them to supervise.
* Participate in the multidisciplinary team within the Dorking Primary care Network Service.
* Adhere to organisational standards for documentation / writing notes.
* Maintain timely patient progress notes on EMIS / Systm1.
* Abide by the record keeping standards of SDHC.
* Involved in service projects and developments.
* Contribute to and support any projects for improving the Community Therapy service.
* Keep clinical skills updated by attending study days and training.
* Maintain personal and professional development.
* Take part in CPD and attend in-service training.
* Participate in SDHC’s appraisal scheme and formulate a personal development plan.
* Participate in Clinical supervision.
* Fulfil any specific tasks as requested by the Team Leader or other therapist.
* Maintain complete confidentiality of all information concerning patients.
* Adhere to all policies and procedures of the Community Therapy Service.
