Main duties of the job

As an Advanced Nurse Practitioner, you will work autonomously to assess, diagnose, treat, and manage patients with a variety of acute and chronic conditions. Youll provide high-quality, evidence-based care through face-to-face, telephone, and online consultations.

Youll be responsible for prescribing medications, ordering and interpreting diagnostic tests, and making appropriate referrals to secondary care or other services as needed. Health promotion, disease prevention, and patient education will also be key aspects of your role.

Working as part of our supportive multidisciplinary team, youll contribute to ongoing service development, clinical leadership, and quality improvement initiatives within the practice.

This role is for 24 hours per week, with some flexibility in working pattern and hours. However, availability on Mondays and Fridays is essential, to support clinic demand and ensure continuity within the team.

Job responsibilities

The following are the core responsibilitiesof the ANP in delivering health services.

* a.Develop, implement and embed health promotion andwellbeing programmes
* b.Manage patients presenting with a range of acute andchronic medical conditions, providing subject matter expert advice
* c.Direct referral to and liaison with consultants and otherprofessional agencies.
* d.Request diagnostic investigations and interpret withinagreed protocols e.g. x-ray, blood tests, scans, etc.
* e.Implement and evaluate individual specialised treatmentplans for chronic disease patients
* f.Identify, manage and support patients at risk ofdeveloping long-term conditions, preventing adverse effects to the patientshealth
* g.Provide advanced, specialist nursing care to patients asrequired in accordance with clinical based evidence, NICE and the NSF
* h.Be aware of duties and responsibilities regarding currentlegislation and adhere to practice policies and procedures on Safeguarding Adultsand Safeguarding Children
* i.Request and collect pathological specimens. Upon receipt, to processand interpret pathology and other test results as required
* j.Maintain accurate clinical records in conjunction withextant legislation ensuring that SNOMED CT codes are used effectively
* k.Develop, implement and embed well woman and well manclinics
* l.Chaperone patients where necessary
* m.Prioritise health issues and intervene appropriately
* n.Support the team in dealing with clinical emergencies
* o.Recognise, assess and refer patients presenting withmental health needs
* p.Be an extended and supplementary prescriber, adhering toextant guidance
* q.Support patients in the use of their prescribed medicinesor over the counter medicines (within own scope of practice), reviewingannually as required
* r.Contribute to practice targets (QOF, etc.), complyingwith local and regional guidance
* s.Liaise with external services/agencies to ensure thepatient is supported appropriately (vulnerable patients, etc.)
* t.Delegate clinical responsibilities appropriately(ensuring safe practice and that the task is within the scope of practice ofthe individual)
* u.Understand practice and local policies for substanceabuse and addictive behaviour, referring patients appropriately
* v.Deliver opportunistic health promotion where appropriate

There may be, on occasion, a requirement to carry out other tasks, this will bedependent upon factors such as workload and staffing levels


Person Specification


Qualifications

* Registered Nurse with the Nursing and Midwifery Council (NMC) (Essential)
* Masters degree (required for ANP qualification post-December 2020) refer to RCN Credentialing for Advanced Level of Nursing Practice (ALNP), or a relevant degree if qualified prior to December 2020 (Essential)
* Qualified Independent Nurse Prescriber on the NMC register (Essential)


Experience

* Understanding of general practice and the wider NHS
* Experience of leading in chronic disease management
* Experience of prescribing and undertaking medication reviews
* Experience of infection prevention and control measures
* Experience of quality initiatives (e.g., benchmarking)
* Clinical Knowledge and Skills
* Understanding of the importance of evidence-based practice
* Ability to promote best practice regarding nursing care and clinical decision-making
* Ability to work within own scope of practice and to recognise when to refer to GPs
* Good clinical IT system knowledge and ability to record accurate, clear clinical notes
* Broad knowledge of clinical governance
* Understanding of safeguarding for both adults and children
* Understanding of health promotion strategies
* Detailed knowledge of national standards that inform general practice (e.g., NSF, NICE guidelines)
* Excellent interpersonal, influencing, negotiation, and organisational skills, including the ability to constructively challenge practices
* Effective time management, planning, and organisation skills
* Demonstrates personal accountability, emotional resilience, and ability to work under pressure
* Ability to follow legal, ethical, and professional guidelines, policies, and codes of conduct
* Ability to identify and manage risk when working with individuals
* Ability to work both independently and as part of a team
* Strong interpersonal and organisational skills
* Sensitive and empathetic in distressing situations
* Knowledge of and ability to work within key policies and procedures
* Appreciation of the NHS landscape, including Primary Care Networks (PCNs), commissioners, and wider system relationships
* Knowledge of public health issues both locally and nationally
* Understanding of QOF (Quality and Outcomes Framework) and enhanced services in general practice
* Knowledge of healthcare provision in GP surgeries

* Working knowledge of IT systems (e.g., email, word processing, internet)

£52,000 to £60,000 a year FTE depending on experience
