Are you a skilled leader who aspires to a rewarding role transforming care pathways? We are looking for an experienced Project Manager, with an interest in system working, to lead work with partners across the Leeds Health & Care system to deliver our local ambition to develop a Neighbourhood Health approach. You would lead delivery of key workstreams in the Active Recovery 2 & Stroke Pathway Programme.

On occasion you may also provide support to a programme testing a new model of integrated, community-based specialist diabetes care.
Roles are offered on a fixed-term basis until the end of March 2027 and may be subject to further extension in 2027/28. We are open to applications from people looking to work between 22.5 and 37.5 hours per week (0.6-1.0 wte).
You will work on behalf of the Leeds Health & Care system, hosted by the Leeds Community Healthcare Business, Change & Improvement Service – a network of skilled and experienced project managers, change agents and business managers. You will work alongside a programme manager and project support officer as part of a smaller team.
