Adults Social Worker Role Overview As part of this service, you will play a crucial role in assessing the needs of adults, facilitating discharge planning and providing ongoing support to individuals and their families. Your help will improve and reduce hospital stay and prevent readmissions. Adults Social Worker Key Responsibilities Conducting comprehensive assessments of patients needs and circumstances. Developing and implementing person-centered discharge plans in collaboration with multidisciplinary teams. Providing emotional support and guidance to patients and their families during the discharge process. Ensuring compliance with relevant legislation and best practice guidelines. Maintaining accurate case records and documentation in accordance with organizational policies. Adults Social Worker Requirements A recognised social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
