Job summary
An exciting opportunity has arisen where we are looking to recruit a Nurse Practitioner to work at our practice as part of our multidisciplinary team consisting of specialist nurses, practice and practitioner nursing, paramedics, pharmacist, gps and hcas, in addition to an admin and finance team and pcn workforce.
The successful applicant will be required to assist medical personnel in the care of practice patients. They will be able to assess, diagnose, plan, implement and evaluate treatment / interventions and care for patients presenting with an undifferentiated diagnosis working both autonomously and as part of a mdt practice.
The nurse practitioner should be competent and confident in managing both new diagnosis and long term conditions and will work at both our Bedford Street Surgery and branch site at Furtzon Medical Centre with the support of our gps.
The practitioner should be able to produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures and deliver care in accordance with guidelines.
A prescriber would be preferred but is not essential. Systm one experience would be preferred but training will be provided.
Main duties of the job
Assist medical personnel in the care of practice patients which include treatment, preventative care, screening and patient education.
Consult with patients to provide assessments, screening treatment services and health education advice and where appropriate refer to other services.
Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate).
Diagnose and manage both acute and chronic conditions, integrating both drug-and nondrug-based treatment methods into a management plan.
Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care.
Regular attendance to shared care and learning meetings and an active participant in team meetings.
Be part of the practice vaccination programmes.

As part of a pcn we also include FCP, health and wellbeing coaches, dietician and mental health practitioners. We have a list size of 15360 patients and our team is growing rapidly to develop a multidisciplinary approach to healthcare .

Job responsibilities
The following are the core responsibilities of the nurse practitioner. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
1. Develop, implement and embed health promotion and well-being programmes
2. Provide a triage service, actively managing or referring patients or treating patients within own scope of practice
3. Support the clinical team in the development of individual treatment plans for chronic disease patients
4. Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health
5. Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
6. Request pathology services as necessary, processing and interpreting results as required
7. Refer patients directly to other services as appropriate
8. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required
9. Maintain accurate clinical records in conjunction with extant legislation
10. Ensure read codes are used effectively
11. Maintain chronic disease registers
12. Develop, implement and embed well woman clinics
13. Develop, implement and embed well man clinics
14. Chaperone patients where necessary
15. Prioritise health issues and intervene appropriately
16. Support the team in dealing with clinical emergencies
17. Recognise, assess and refer patients presenting with mental health needs
18. Recognise, assess and refer patients with community health needs, developing strategies for effective care pathways
19. Implement vaccination programmes for adults and children

22. Support the clinical team with all safeguarding matters, in accordance with local and national policies
23. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
