Job summary
The Cancer Services department at Frimley Park Hospital is a busy service comprised of Tumour Site Specific Multi-Disciplinary Teams.
We are looking for a highly motivated and proactive individual, who has experience of working within a team environment, who will virtually navigate patients through their care pathway from point of referral to treatment in accordance with agreed pathway steps and timescales, proactively identifying and resolving delays at all stages. They need a strong understanding of the two week wait, 28 day, 31 day and 62 day targets, will ideally have previous NHS experience, along with excellent IT, communication, interpersonal skills and the ability to prioritise and organise their own work with minimal supervision.
The successful applicant will be a person that embodies the Trust Values and has the competencies to fulfil the core role, which requires the ability to efficiently and effectively co-ordinate tumour site specific Multi - Disciplinary Team meetings, track patients against Cancer Waiting Times targets to ensure Minimum Data Sets and Audits are completed, and capturing and accurately recording the relevant data for each patient in line with the National Data requirements.

Main duties of the job
1. To actively track patient's progress through the patient pathway to ensure that care is carried out in a timely fashion on daily basis and follows escalation and tracking process on PTL (Patient Tracking Tool).
2. Specific co-ordination and actioning of process of cross site referrals to another specialty (internal within the Trust)
3. General co-ordination and actioning of MDT - eg. Tracking patients post MDT, updating with requested investigations, referrals to tertiary centres, and general pathway progress on SCR
4. To facilitate multidisciplinary team meeting discussion and ensure cancer data is captured and recorded on Somerset
5. To ensure that all cancer patients are identified and discussed at MDT meetings.
6. To ensure patients' diagnoses, investigations, and management and treatment plans are recorded on Somerset to ensure cancer data uploaded is accurate.

Job responsibilities
7. To ensure patients have a booked 1st appointment, investigation and procedure and record details of patients coming via a different route on appropriate electronic systems.
8. To work with key MDT members to identify areas where cancer waiting time targets are not achieved, undertake process mapping to identify bottlenecks.
9. To collect and record cancer waiting time and clinical data and report potential or actual breaches.
10. To train with service improvement methodology for Cancer MDT coordinators and other MDT members.
11. To establish effective communications networks with clinicians and other members of the multidisciplinary team, and actively participate in development and change to any systems to improve patient journey.
12. To use clinical systems according to guidelines, monitoring milestones and submitting the required reports in the given format and required times.
13. To keep comprehensive electronic diary of all team meetings including attendance.
14. To accurately capture the MDT outcomes and record on SCR staging of CT's, performance status COSD and relevant national audits etc
15. Maintaining own personal and professional development, by participating in appraisal process and accessing training commensurate with role as highlighted by team leader/manager.
16. Ability to communicate with all levels of staff including senior clinicians.
17. Work collaboratively with clinical admin team within the trust to ensure appropriate management of patient information and co-ordination of patient pathways.
18. Effective management of own time and resources.
19. To be proactive in the pathway management of all suspected and diagnosed cancer patients through from referral to treatment.
20. Understanding the risk of potential breaches and the impact from a patient, Trust performance and financial perspective and the need to provide evidence to enable validation of and uploading of completed Cancer Breach Reports.
21. To provide cross-functional cover for all multidisciplinary team meeting.
22. Work with Pathway Administrators to ensure all patients tracked effectively
23. Facilitate Video Conferencing.
