A rare and excellent opportunity has arisen for a Band 5 Occupational Therapist to join an established, motivated, and forward-thinking Adult Learning Disabilities Occupational Therapy Service.
This post is for 30 hours a week (0.8 WTE). An Occupational Therapist is required to work within the Community Learning Disability Team, providing proactive support to individuals with Learning Disabilities and additional health needs.
This role offers the opportunity to work within a multidisciplinary team approach, in various community settings, providing specific healthcare interventions. Essential skills include effective communication and organization; we welcome applications from candidates who demonstrate confidence, self-awareness, and enthusiasm for working with this client group.

We offer flexible working arrangements and a preceptorship programme where applicable. A full driving licence and access to a vehicle are essential for this post.
Responsibilities:
1. Carry out the Occupational Therapy process (information gathering, assessment, treatment planning, intervention, evaluation, and discharge planning) using a person-centred approach.
2. Work flexibly within the multidisciplinary team to provide a holistic approach to meet patient and service needs.
3. Offer education, advice, and training to informal and formal carers to support patient safety.
4. Ensure patient information is accurately collated, monitored, and maintained in line with HCPC/professional standards and Trust policies.
5. Collaborate closely with multidisciplinary colleagues and other professionals within and outside the MDT.
6. Maintain knowledge of professional standards, codes of conduct, and ethics.
7. Maintain accurate record keeping and report writing skills.
8. Previous experience working with this patient group is desirable.
