Hello Everyone ,\n\nPlease find the Job Description for the Part-Time Care Coordinator role below:\n\nJob Title: Part-Time Care Coordinator\n\nLocation: South East London (SE13)\n\nHours: Part-Time, 24 hours per week (3 days)\n\nWorking Days : Mondays, Fridays, and 1 day every weekend (2 Saturdays & 2 Sundays per month)\n\nShift :10:00 AM - 6:00 PM\n\nSalary : £15,238.08 per annum / £12.21 per hour\n\nHybrid Working : 2 days in the office, 1 day working from home\n\nAbout the Role:\n\nLEO Recruits is seeking a dedicated and organized Part-Time Care Coordinator to join our client's domiciliary care agency based in South East London (SE13). This is an excellent opportunity to contribute to the delivery of high-quality care services within the local community. The role is part-time, working 24 hours per week across three days, with a hybrid working model offering flexibility.\n\nKey Responsibilities:\n\n1.

Care Planning: Develop, implement, and review personalized care plans with clients, families, and healthcare professionals.\n\n2. Client Assessments: Conduct initial assessments, risk assessments, and ongoing evaluations to identify client needs.\n\n3. Care Scheduling: Coordinate and schedule care visits, ensuring timely and efficient service delivery.\n\n4.

Career Allocation: Match care workers with clients, considering compatibility, skills, and availability.\n\n5. Communication: Liaise with clients, families, care workers, and healthcare professionals to ensure seamless care delivery.\n\n6. Record Keeping: Maintain accurate, up-to-date records, including care plans, visit logs, and client information.\n\n7.

Quality Monitoring: Monitor care quality, addressing concerns and implementing improvements.\n\n8. Compliance: Ensure adherence to regulatory requirements, company policies, and procedures.\n\n9. Team Support: Assist with care worker training, supervision, and performance management.\n\n10.

Emergency Response: Participate in on-call Rota (if required) and respond to care-related emergencies.\n\nEssential Skills and Qualifications:\n\n1. Qualifications: Diploma/NVQ Level 2/3 in Health and Social Care or equivalent.\n\n2. Experience: Minimum 1 year in domiciliary care coordination or related field.\n\n3.

Communication: Excellent communication, interpersonal, and organizational skills.\n\n4. IT: Proficient in Microsoft Office, care management software, and scheduling systems.\n\n5. Problem-Solving: Strong problem-solving and analytical skills.\n\n6.

Time Management: Ability to priorities tasks and manage time effectively.\n\n7. Knowledge: Understanding of relevant legislation, regulations, and care standards.\n\nDesirable Skills and Qualifications:\n\n1. Additional Qualifications: Qualifications in care management, nursing, or social work.\n\n2.

Specialized Training: Training in dementia care, palliative care, or other specialized areas.\n\n3. Language Skills: Proficiency in additional languages.\n\n4. Experience: Experience working with vulnerable adult groups (e.g., learning disabilities).\n\nWorking Conditions:\n\n1.

On-Call: Participation in on-call Rota may be required.\n\n2. Travel: Occasional travel to client locations, meetings, and training events.\n\nInterview Process:\n\n1. Leo Recruits Telephone Pre-Screen\n\n2.
