Pathways Team
This service operates through Pathways Hubs and provides day services for young people.
Key duties and services include:
* Person-Centred Care and Support: Delivering high-quality, person-centred care to improve and maintain an individual's health, well being, and happiness.
* Therapeutic Activities: Offering a wide range of enriching and multi-sensory therapeutic interventions.
* Meaningful Activities and Education: Facilitating educational, social, and enjoyable activities
* Specialist Support: Working closely with healthcare professionals (e.g., Physiotherapists, Occupational Therapists, Speech and Language Teams) and undertaking specialist tasks
* Focus on Outcomes: Supporting individuals to develop skills, promote self-confidence, build friendships, and work towards goals related to employment, independent living, community inclusion, and health.
Qualifications and requirements
* Social work qualification
* Registered with SWE
* Full Driving License
