Overview
Dorset HealthCare are currently seeking a Community Staff Nurse to join our Integrated Care Team in Purbeck and the District and Community Nurse team in Wareham. You would work alongside experienced Community Nurse Teams helping to provide care to a high-quality standard for patients in their own homes. We can be flexible with shift patterns as long as the service needs are met between 08:00 and 20:00. We offer a structured induction programme, flexible working patterns and pastoral support, with opportunities for learning and development both in-house and through links with local universities, plus the option of a preceptorship programme for newly qualified nurses.
Employment requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover. Applicants who subscribe to the DBS update service may present a valid DBS certificate instead of a new check. Applications from candidates requiring current Skilled Worker sponsorship to work in the UK are ineligible for this position.
Main duties
Main duties are to provide a high-quality, person-centred approach to care delivery that always considers safety, privacy and dignity while promoting the Trust\'s values. Undertake assessment of patients\' care needs and develop programmes of care as part of the wider multi-disciplinary care team. Manage acute and chronic nursing needs, build strong therapeutic relationships with patients, and work with community partners to achieve the best possible health outcomes. Identify care needs, monitor patients\' conditions, record observations and help develop wellbeing care plans. Support people to maintain wellbeing and independence, help to avoid unnecessary hospital admissions, and deliver exceptional end-of-life care including symptom management and helping to identify and achieve patients\' preferences.

Qualifications and Experience
Knowledge, Skills and Training
* Registered practitioner to degree/diploma level
* Membership of the relevant Professional Body
* Evidence of recent professional development in an up-to-date portfolio
* Knowledge and understanding of Trust Strategy and policies relevant to the role

Job Specific Experience
* Essential: Supervised experience gained during clinical placements
