<div><p>The Community Resource team are looking for an enthusiastic person to join our service. The role is a busy one and will involve carrying out functional assessments in a hospital or community setting, completing IAs and Care and Support Plans and undertaking reviews for packages of care. The role will also involve building and maintaining positive working relationships with our health colleagues. We need someone who is flexible hard working, and who can work at pace, as well as being motivated and enthusiastic about helping individuals to find solutions to achieve what matters to them and achieve their identified outcomes.</p><ul><li>Level 2 qualification/s on the Credit and Qualification Framework for Wales. For example O Levels Grade A-C, NVQ Level 2 in Care</li><li>Knowledge of social services and hospital interfaces and need for community based care</li><li>A Full UK Category B (Cars) driving licence and the use of a motor vehicle insured for business / work purposes to travel throughout the borough to visit clients at their home, attend meetings in various locations.</li><li>We have excellent benefits including the Local Government Pension Scheme and staff discount schemes.</li></ul></div>
