Specialist Palliative Care Nurse

To act as an expert nurse for patients and families and be a resource for advice and consultancy to empower other professionals to provide high quality palliative/end‑of‑life care. To work autonomously to provide holistic assessment and management for patients with complex problems and their families, enhancing quality of life and enabling them to make choices. To monitor, evaluate and review palliative interventions. To act as a patient advocate and support informed discussions around choices concerning treatment options affecting quality of life and preferences around place of care and place of death. To work collaboratively as part of a multi‑disciplinary team supporting colleagues and respecting their contribution to meetings. To have organisational knowledge relating to Trust protocols and procedures and adhere to them. To be responsible for providing accurate records of information required by the Trust. To contribute to quality initiatives and service development. To have sound clinical knowledge of all aspects of palliative care and palliative medicine, with awareness of the benefits and reactions to treatments and drugs used for symptomatic and therapeutic benefit, providing expertise in symptom control and offering appropriate advice as required.
Responsibilities

* Advocate for patients and support discussions on treatment options and preferences for place of care and death.
* Collaborate with a multi‑disciplinary team and respect colleagues’ contributions.
* Use Trust protocols and procedures correctly and maintain accurate records.
* Contribute to quality initiatives and service development.
* Apply sound clinical knowledge of palliative care and medicine, handle symptomatic and therapeutic treatments, and advise on symptom control.
Qualifications
* Good standard of literacy and numeracy.
* Advanced communication skills.
* Training or commitment to undertake teaching qualification.
Experience
* Two years or longer working at a senior level (Band 6 and above) in a hospital or community setting.
* Experience of teaching and mentoring/coaching staff.

Additional Information
* Probationary period applies.
* High Cost Area Supplement – Outer London: 15% of basic salary (minimum £4,870, maximum £6,137 per annum, pro rata for part time).
