* Acting as the first point of contact for new and existing clients, conducting risk assessments, care plans and ongoing care reviews, for our clients in the community
* Ensuring time sensitive assessments are prioritised, so that support can be provided efficiently.
* Ensuring where appropriate, adaptations or equipment has been adequately assessed and is continuously monitored, this may mean escalation to other health professionals where necessary
* Maintain accurate record keeping and sound governance of processes and procedures
* Working in a busy office environment, answering and recording calls.
It is essential you have experience within a Health and Social Care setting, providing robust and detailed Risk Assessments and Care Plans, with a focus on a person-centred care approach.
Working alongside a Kind, caring and compassionate team, you will form a key part of the ongoing success and growth of the team.
The role is a part - time with potential of leading to a full time position, Monday, Wednesday and Friday.
You will be required to be on-call, 1 weekend per month plus 1 day per week for 3 out of 4 weeks. This may involve providing care to our clients in the event of sickness.
