An exciting opportunity has arisen for a Community Psychiatric Nurse, Band 6, to join the team on a permanent basis in Thurrock, Civic Offices New Road, Grays. You will be employed as a Care Co-ordinator within a Multi-Disciplinary Team, working with patients with Severe and Enduring Mental Health Illness, including diagnosis of Alzheimer’s and dementia and early onset dementia.
We are looking to appoint a dynamic and suitably experienced Mental Health Practitioner who is flexible and able to work on their own initiative, as well as part of a Multi-Disciplinary Team. You will have experience of undertaking comprehensive assessments, including physical health care, health promotion, social inclusion and recovery. The post holder will be responsible for supporting and reviewing the care needs of patients with mental health and dementia within the Older People’s Community Mental Health Team. A key aspect of the role will be to support patients in their homes, working closely with their families and carers, facilitating discharge from hospital and working in partnership and collaboratively with other services.
You should have experience of inter-agency working, be competent in prioritising workload in a busy environment and have a proven ability to problem solve and deal with everyday practice issues. Responsibilities
* Act as the named care co-ordinator and manage the coordination of care needs, planning and implementation of evidence‑based care for a defined group of service users and their carers, within Thurrock.
* Provide professional advice and support to other agencies and members of the multidisciplinary team, and act up for the Team Lead in their absence as appropriate.
* Be a role model and resource for junior/less experienced staff.
* Manage caseload and work independently with the community, with an awareness of safeguarding principles.
Qualifications and Experience
* Experience of undertaking comprehensive assessments, including physical health care, health promotion, social inclusion and recovery.
* Experience of working in an Older People’s Community Mental Health Team and with patients with mental health and dementia, including Alzheimer’s and dementia.
* Experience of inter-agency working, ability to prioritise workload in a busy environment, and strong problem-solving skills.
