We are currently recruiting – Service Co‑ordinator.
We are looking for a creative and resourceful Social Care Service Co‑ordinator to work with the Upper Valley Locality Team, which is part of the Adult Services and Wellbeing Directorate at Calderdale Council. The locality teams support people who have already been assessed and have support in place; we manage reassessment where needs change or review, and provide person‑centred support for care‑act‑eligible individuals. The team primarily supports people with a physical disability and those over the age of 65 with age‑related needs. Physical disability work includes transition support with children services to aid a seamless move from children to adult services.
Within the role you will be expected to carry out Care Act Assessments and relevant support planning, mental capacity assessments, support with court‑work processes on allocated cases, hospital discharge work, continuing health care, duty function and reviews.
We work in partnership with both statutory and voluntary sectors to ensure a person is sign‑posted to services that best suit their needs and to consider a holistic approach to support planning. We use strengths‑based practice focussing on positive outcomes and independence, putting the person at the heart of assessments.
What we are looking for:
Ability to relate to people
Your role as a Social Care Service Co‑ordinator is to have direct contact with people, usually on a face‑to‑face basis. You must be able to develop a trusting relationship quickly, using a kind, empathetic and friendly approach. You should listen deeply to what people are telling you and adapt your communication style to get the best out of the interactions.
Resourcefulness and creativity
Formal services are not always solutions. You will need to be useful, resourceful and creative, drawing on the strengths of people, their wider networks, and local community assets to problem‑solve and meet their requirements. You should be confident to challenge the status‑quo or preconceived ideas to enable this to happen.
Adult Social Care knowledge
The ideal candidate will have experience and knowledge of adult social care. Whether you have lived experience, have worked as a care worker or are in a similar role, your background will provide an excellent platform to fulfil the duties of a Social Care Service Co‑ordinator.
