The post holder is responsible for delivering basic practice nursing services to the practice population. Supported by senior nurses, they will provide care within their role, focusing on supporting patients to be healthy, monitoring long-term conditions, and conducting health prevention and screening activities. They will collaborate with the general practice team to meet patient needs, support policy and procedures, and provide nurse leadership as required.
Key Responsibilities:
1. Promote health and well-being, and prevent adverse effects on health
2. Implement and evaluate treatment plans for patients with long-term conditions
3. Identify and manage treatment plans for patients at risk of developing long-term conditions
4. Prioritize health problems and intervene appropriately in complex, urgent, or emergency situations, including emergency care initiation
5. Support patient adherence to prescribed treatments
6. Encourage health promotion strategies and self-care among patients
7. Deliver opportunistic health promotion during activities such as new-patient medicals
8. Assess and care for patients with uncomplicated wounds
9. Support women during cervical cytology consultations
10. Participate in vaccination and immunization programs for both adults and children
11. Administer vaccinations for patients traveling abroad where appropriate
The ideal candidate will be an exceptional team player with good communication and interpersonal skills, enthusiastic, and committed to joining the team. Experience in primary care, chronic disease management, diabetes, first contact, and telephone triage is preferred. Candidates must possess a practice nurse qualification with at least two years of experience; experience with long-term conditions is desirable.
