B5 Social Worker with Recruitment Incentive
Join Hertfordshire Partnership University NHS Foundation Trust (HPFT) as a Band 5 Social Worker in our adult community mental health service in Bishops Stortford.
Responsibilities
* Provide high‑quality social care to adults facing mental health challenges, using a recovery framework that prioritises individual choice and control.
* Carry out assessments, develop and implement care plans, and coordinate services across a multidisciplinary team.
* Support and assess carers as part of your caseload.
* Manage a caseload, coordinate care, deliver specific interventions, and maintain flexibility and a positive professional approach.
Qualifications
* Social Work qualification, either newly qualified or with experience.
* Strong interest in mental health issues and understanding of the Care Programme Approach, Care Management and Safeguarding.
* Experience with adult community mental health services is desirable.
* Support for newly qualified Social Workers through the ASYE programme is available.
