Main duties of the job

* Conduct nurse-led clinics for long-term condition reviews (e.g. diabetes, COPD, hypertension)
* Deliver immunisation and vaccination programmes for all age groups
* Support health promotion and screening initiatives (e.g. smoking cessation, NHS Health Checks)
* Perform venepuncture, ECGs and point-of-care testing
* Assess and manage leg ulcers, applying and reviewing complex dressings
* Lead and execute comprehensive infection control processes and audits
* Maintain accurate clinical records and ensure adherence to practice protocols
* Collaborate with GPs, HCAs and other team members to coordinate holistic patient care.
* Arrange for and perform appropriate tests as indicated such as blood, swabs etc.
* Make referrals as necessary to other health and social care professionals
* In participation with the Practice Manager, ensure the maintenance of equipment and stock levels relating to patient care
* Undertake the collection, storage and despatch of pathological specimens including intravenous blood samples, swabs, smears etc. and ensure the cleanliness and stocking of equipment used
* Perform any investigatory procedure requested by the GPs such a spirometry., ECGs, 24 hour blood pressure monitoring etc.
* Ensure accurate completion of necessary documentation associated with patient health care or registration, supplies, equipment calibration etc.
* Attend and participate in practice meetings as required

Job responsibilities

1. Role Purpose

As PracticeNurse at Longcroft Clinic, you will deliver high-quality, patient-centrednursing care across a broad spectrum of clinical services. You will leadnurse-led clinics, manage long-term conditions, perform specialist procedures,and champion infection-control standards. Working closely with GPs, nursingcolleagues and healthcare assistants, you will be instrumental in driving serviceimprovements, mentoring junior staff, and ensuring our patients receiveseamless, evidence-based care.

2. Key Responsibilities

* Chronic Disease Management
* Plan and run nurse-led clinics for diabetes, COPD, hypertension,CKD and other long-term conditions
* Review clinical parameters, interpret results, and adjusttreatment plans in collaboration with GPs
* Deliver childhood and adult immunisation programmes (includingflu, travel and catch-up vaccinations)
* Maintain cold-chain documentation and safety protocols
* Wound Care & ComplexDressings
* Assess and manage acute and chronic wounds, including leg ulcers
* Select and apply advanced, complex dressings; monitor healing andmodify care plans
* Infection Control &Audit
* Design, implement and lead comprehensive infection-control auditsand action plans
* Maintain and validate sterilisation/decontamination equipment;ensure full CQC compliance
* Clinical Procedures &Point-of-Care Testing
* Perform phlebotomy, ECGs, spirometry and finger-prick tests (e.g.INR, HbA1c)
* Health Promotion &Screening
* Conduct NHS Health Checks, cervical cytology and lifestyle-adviceconsultations
* Support smoking-cessation clinics, weight-management programmesand health-education events
* Maintain accurate, contemporaneous patient records in EMISWeb
* Participate in clinical-governance meetings, significant-eventreviews and practice-wide audit cycles
* Mentor and supervise HCAs, trainee nurses and student placements

3. Person Specification

Registered Nurse (NMC Part 1) with valid PIN

Minimum 2 years experience in UK primary care

Proven competency in chronic-disease management

Wound-care certification (e.g. Wound Care Lead) (desirable)

Skilled in infection control processes, audit andequipment validation

Experience with EMIS Web clinicalsystems

Strong communication, organisational and ITskills

Leadership, mentoring or project-managementexperience

Flexible, proactive, patient-centred, andteam-oriented approach

Interest in service-development and QI (QualityImprovement)

Qualifications

* Professional Qualifications & Registration
* Registered General Nurse (RGN/RMN) on NMC Part 1 with an active PIN
* Evidence of valid and up-to-date annual NMC revalidation
* Clinical Experience
* Advanced Qualifications & Skills
* V300 Independent/Supplementary Prescribing qualification
* Wound-care certification (e.g. Tissue Viability or Wound Care Lead course)


Experience

* Minimum 1 years post registration experience in UK primary care or comparable community setting
* Proven track record managing chronic disease clinics e.g. diabetes, COPD, hypertension
* Demonstrable experience in wound care, including assessment and complex dressing application
* Experience leading service-improvement or quality-improvement projects in a primary-care setting
* Demonstrated ability to deliver presentations or training sessions to clinical teams
