Job Overview
The post holder will work as part of the specialist diabetes team, as an autonomous practitioner in the provision of evidence based and high‑quality diabetes care within the diabetes inpatient team. Ensuring good quality consistent care for people with diabetes and to enhance the care of patients with diabetes across all pathways.
Key Responsibilities
* Deliver a seamless service through the development of enhanced MDT processes and communication.
* Contribute and provide high quality training for patients, staff and carers.
* Develop and enhance the skills and knowledge required for managing diabetes effectively.
* Support and develop with colleagues the promotion and development of services for diabetes patients across the primary/secondary interface and throughout the pathways.
* Have relevant expertise, skills including prescribing and knowledge related to possible investigations and treatment options and so to provide the patient with the necessary information to make informed choices.
* Where appropriate prescribe appropriate treatment as an independent non‑medical prescriber.
* In conjunction with the diabetes specialist nurse team, ensure the provision of a comprehensive nurse‑led service for patients with diabetes.
