MAIN DUTIES & RESPONSIBILITIES:
Main Duties and Responsibilities:
* To be the first point of contact for referrals where concerns have been identified over an individual's mental
health
* Work in a trauma informed way to assess individual mental health support needs and wishes.
* Develop effective person centred inter-disciplinary care & support plans for your caseload of clients
* Work in partnership with other agencies supporting clients – i.e. Bedford Borough Housing/ Health/ MIND/
P2R etc.
* Provide follow-up support to maximise tenancy sustainment and reduce the risk of returning to the AP
* Maintain accurate records, provide case-studies and report outputs, outcomes and progress quarterly
* Support residents to attend appointments – including at points of crisis, making applications that may improve
mental wellbeing where appropriate
* Monitor and review progress, amending the support plan where necessary and keeping accurate records
* To travel across the borough to meet with residents as required.

Experience & knowledge:
Experience of supporting people with a range of mental health conditions and working in partnership to achieve best outcomes

Sound understanding of the impact of complex trauma on mental & physical health outcomes
Y
A good knowledge of the issues facing people who have experienced domestic abuse and/ or homelessness issues in general.

Good understanding of Trauma Informed Care as part of a Psychologically Informed Environment (PIE)

Good understanding of the Safeguarding Agenda

Education and Qualifications:
Qualification in mental health, psychology, or related social care discipline

Evidence of relevant CPD

Skills and Abilities:
The ability to be tactful and sensitive in communication and to empathise

Ability to stay calm and 'think on your feet' in crisis situations
Y
Ability to build strong working relationships with clients while maintaining professional boundaries

Excellent written and verbal communication skills with the ability to engage and communicate with people at all levels.

IT proficient in Excel, Outlook, Word etc.

SCHEDULE:
37.5 hours a week Monday to Friday 9:00AM-5:00PM – willing to be flexible on occasions
