We are looking for a motivated, newly qualified individual who wishes to work with clients with diverse and complex mental health needs. Main duties of the job include:
1. Assessing, planning, implementing, and evaluating evidence-based care, including health promotion for a designated service user or group.
2. Conducting clinical risk assessments for service users contacting the Out of Hours service, providing advice, guidance, and decision-making, and coordinating care plans.
3. Managing admissions, case conferences/reviews, and discharges in collaboration with the multidisciplinary team.
4. Coordinating care and assessments under the Care Programme Approach for a caseload.
5. Participating in service development, setting quality standards, and conducting audits in line with current clinical guidance, practice, and policy.
6. Ensuring understanding and compliance with Safeguarding Vulnerable Adults and Children Legislation.
7. Adhering to the requirements of the Mental Health Act 1983.
8. Promoting carer and service user involvement within the service.
9. Establishing therapeutic relationships and implementing evidence-based therapeutic interventions with professional boundaries.
10. Ensuring carers' assessments, education, and support are delivered per NSF requirements.
11. Providing support and coverage for colleagues and team members as needed.
12. Completing mandatory training, participating in staff appraisals, and staying updated on trust policies.
13. Providing management supervision where appropriate.
14. Collaborating with key partners, service users, carers, clinicians, and practitioners within the multidisciplinary team.
