We are looking for compassionate, motivated practitioners with a passion for person-centred care to join our multidisciplinary Community Admissions Avoidance Therapy Team (CAATT) at the Royal Gwent Hospital.
Main duties of the job
The CAATT service is based at the hospital's 'front door' and provides rapid response assessments and interventions for adults, aiming to facilitate home care and avoid unnecessary admissions. When admission is necessary, CAATT initiates early therapy interventions and guides patients to appropriate ongoing care, promoting timely discharges and reducing hospital stay lengths and readmissions.
The post holder will be a Physiotherapist supervised by a Highly Specialist Physiotherapist and will work closely with a Highly Specialist Occupational Therapist. They will work across disciplines, including Dietetics, Occupational Therapy, Physiotherapy, and Speech and Language Therapy, to deliver holistic, prudent interventions.
