To provide high quality nursing care to patients on the Sutton Health & Care (SHC) Reablement Unit as part of a cohesive Multi-DisciplinaryTeam.
The post holder is responsible for holistic assessment and care needs of patients, including planning, implementation and evaluation.
In addition, the post holder will be involved in health promotion and innovative activities that enhance patient service delivery and the patient experience.
The post holder will at times have management responsibility such as Nurse In Charge and supervising junior members of staff.
This post is not suitable for a newly qualified nurse.

* Undertake comprehensive assessment of the physical, psychological and social careneedsofpatientsandprovidepersoncentredcareplansthatfacilitatethepatient’sownchoices,promotingindependenceand selfcare,as appropriate.
* Toensurethesafehandlingofbodyfluidsandcontaminatedsharps.
* Be alert to the needs of vulnerable adults including risk assessment and taking appropriateactionwhen required.
* Safelyadministerprescribedmedicationsandtomonitortheireffects.
* Usereflectivepracticeasamethodofensuringthatappropriateeffectivenursingcareisdeliveredto eachpatient.

* Workwithnursingandother healthcareprofessions,patientsandtheirrelatives/carerstoassist in the timely discharge from the clinical area through appropriate dischargeplanningin conjunction with theDischarge Co-ordinators.
