Job Description:
We are looking for an experienced and passionate Care Navigator to work as part of our Prisons and Criminal Justice Directorate Team.
Imagine being part of an organisation whose common purpose is to help those who are severely impacted by mental illness. We believe that everyone should be treated with respect and dignity – and that’s why equity is one of our core values. We draw on the expertise, unique perspectives and lived experience of our people – regardless of who they are or their background – to help us become an inclusive and anti-racist employer, campaigning organisation, and service provider that reflect the diverse communities we support as a mental health charity.
Rethink Mental Illness works alongside Tees, Esk and Wear Valley NHS Foundation Trust (TEWV) and Humber Teaching NHS Foundation Trust to deliver an integrated stepped care mental health service within HMP Hull and Humber.
The team comprises a range of highly skilled professionals who work collaboratively to provide the highest quality of care for those with mental health needs within the prison. For Rethink Mental Illness employees, this is carried out between 0800 – 1600 Monday to Friday.
There may be some travel involved, but this will be done during work hours and expenses paid to support service delivery.
How you will make a difference
I provide a seamless package of support to ensure holistic and meaningful support is provided to those with health vulnerabilities on their release from prison, ensuring continuity of care and fruition of release plans. Reconnecting individuals with health services to reduce health inequalities and in turn reduce reoffending. The Care Navigators are embedded in integrated mental health teams and community settings, providing 1:1 and group assessment and support in a variety of settings and hold highly autonomous roles. Care Navigators use a coaching framework to manage wellbeing in the pre and post-release periods, to prevent a return to ill health and reduce reoffending. By preventing health decline through robust reconnection with community health services, we aim to reduce health inequalities for those leaving the criminal justice system and support a seamless, effective transition to independent community living.
Diversity, Equity, Inclusion
We value diversity and believe it enhances our understanding of the world, society, and the communities we serve. We support staff networks for ethnically diverse and LGBTQIA+ colleagues and are proud to be a Disability Confident Employer and signatory to the Race at Work Charter. We aim for our workforce to reflect the communities we serve, and we actively encourage applications from individuals with lived experience of mental illness, from diverse ethnic backgrounds, and those with disabilities or neurodiversity, regardless of age, religion, or other characteristics.
Becoming a truly anti-racist organisation is a core ambition. We have a Race Equality Action Plan outlining our commitment and progress in this area. More details can be found on our website.
