Clinical Lead

This is a full‑time Community Nurse, Band 6 position working 37.5 hours per week between the hours of 8 am – 8 pm.

Based in Ellesmere Port, the post involves visiting patients in the Ellesmere Port and Neston areas. Travel is an essential part of this role and access to a car for work purposes is required.

Main duties of the job

The post holder will be a clinical resource, providing a visible clinical profile, having advisory, leadership and clinical oversight of the caseload.

The team focus is on prevention of unnecessary acute hospital admission, end‑of‑life care and facilitating discharge from hospital.

Working collaboratively with the wider healthcare team ensures person‑centred care for those patients whose preferred place of care is at home.

A high level of communication is essential, with teamwork at the core of our practice.

Person Specification

Qualifications

  • First level registered general adult nurse
  • Evidence of teaching and assessing
  • SPC qualification in District Nursing or equivalent; demonstrating robust evidence of caseload management, community profiling and application of health needs assessment

Knowledge and Expertise

  • Proficiency in delivering high quality evidence based care
  • To be able to demonstrate continuing professional development
  • Ability to work and communicate effectively within a MDT setting

Experience

  • Significant experience of working within a community healthcare setting
  • Experience of making autonomous decisions
