Job Overview

Avonside Health Centre is a friendly GP Practice. Patient care is at the forefront of everything we do. We are looking for a new part-time Care Navigator to join our diverse team under the Additional Roles Reimbursement Scheme, and to help promote our values as a Practice. This role is for 31 hours a week working:

* Mondays 8.15 to 13.00
* Tuesdays 8.15 to 18.00 with a 1 hour unpaid lunch break
* Thursdays 8.15 to 18.00 with a 1 hour unpaid lunch break
* Fridays 8.15 to 18.00 with a 1 hour unpaid lunch break

The Care Navigator role ties all aspects of patient care together including assisting patient queries and booking appointments to processing secondary care communications. The key focus is to improve a patients journey and experience to ensure they receive the best care possible.

Responsibilities

* Covering front-desk and answering/initiating telephone calls in order to facilitate timely and appropriate communications with others, taking messages and dealing with appropriate queries
* Booking appointments with a variety of clinicians and Allied Health Professionals
* To provide an efficient administration service for GPs. This includes the processing and booking of secondary care referrals/letters, typing minutes and actioning emails
* Supporting staff with general administrative tasks as requested
* To liaise, arrange, attend and minute meetings as required, including multi-disciplinary team and palliative care meetings
* Supporting clinical staff with general administrative tasks such as laboratory requests and prescription requests
* Assist management with clerical and administrative duties including recalls, coding and minutes
* Clinical data audits and searches to support and provide recalls and care planning
* To provide cover for colleagues during periods of sickness and annual leave.
* To code relevant data on Practice systems
* Support patients to access services by ensuring service information is up to date, working with Allied Health Professional to support and develop this

Experience

* Previous office experience is desirable for managing administrative duties effectively.
* Experience of working in a customer-service type role is essential
* Strong communication skills, both verbal and written, are necessary for interacting with patients and healthcare professionals.
* Experience in a healthcare setting is advantageous but not mandatory.
* Proficiency in using office software and electronic health record systems is preferred
* A compassionate approach to patient care is vital, along with the ability to work collaboratively within a team environment. This role offers an opportunity to make a meaningful impact on patients' lives while developing valuable skills within the healthcare sector.

Job Types: Part-time, Permanent

Pay: £12.79 per hour
