An exciting opportunity has arisen at Mersey and West Lancashire Teaching Hospital NHS Trust for a full-time band 6 Cardiac Rehabilitation Nurse.
We are looking for a dynamic, forward thinking, highly motivated individual to join our established Cardiac Rehabilitation team.
Main duties of the job
As part of the team, you will be responsible for providing evidence based Cardiac Rehabilitation to patients following MI's, PCI's, post cardiac surgery, a diagnosis of Heart Failure or insertion of a device.
You will deliver complex educational talks to patients and carers, have an in-depth knowledge of cardiac conditions and facilitate in the delivery of high-quality individualised care for patients accessing the service whilst encouraging lifestyle changes beyond the acute phase.
You will be supporting a Nurse Led Service, ensuring the effective delivery of Cardiac Rehabilitation on a day-to-day basis, adhering to the National standard for the delivery of Cardiac Rehabilitation (NICE/ BACPR) and take part in the National Audit of Cardiac Rehabilitation (NACR). You will work collaboratively with Consultants, GP’s, Primary and Secondary care teams, and other hospitals referring patients for Cardiac Rehabilitation ensuring a seamless service.
The post holder must have excellent communication and interpersonal skills, be able to work autonomously and as part of the MDT. You will be able to identify changes in a patient’s condition and deliver ILS if required.
As part of the team, you will be expected to support improvements in service delivery. Promote and maintain a learning environment, identify learning opportunities, and have a desire for ongoing professional development.

We provide the Mersey Regional Burns & Plastic Surgery Unit at Whiston Hospital and the Spinal Injuries Unit at Southport Hospital to more than 4 million people across the whole of Merseyside, West Lancashire, Cheshire, Isle of Man and North Wales.
