Short-term Intervention Team Lead Worker

We are looking for a Short-term Intervention Team Lead Worker to join our Short-term Intervention Team working 12hrs day/night shifts on a 4 on / 4 off basis.

Derbyshire Healthcare NHS Foundation Trust, on behalf of Joined Up Care Derbyshire (JUCD), has funded a ‘Short-term Intervention' pilot for people aged 18+ with learning disabilities and/or autism who live independently, receive little or no support, and may be reaching a crisis point or at risk of a mental health hospital admission or family/carer breakdown.

The aim of this service is to provide a crisis-intervention service for people with LD & ASC who are at risk of otherwise being admitted to a mental health inpatient setting. The service is part of the local hospital avoidance pathway to prevent clinically inappropriate hospital admissions and to provide credible alternatives in the community.

The service works closely with the local Intensive Support Team (IST) and the Specialist Autism Team (SAT) to deliver non-clinical interventions to support service users while clinical support is mobilised and delivered. It is available across Derby and Derbyshire for 365 days per year, on a 24/7 basis, with support mobilised to a person within 2 hours of their referral being accepted.


Key Skills and Experience

* Excellent communication skills
* Ability to manage own time effectively with a keen eye for detail and to complete tasks to the highest quality
* Demonstrated safeguarding knowledge and application
* Experience supporting individuals with ASC and/or LD, and their families/carers and professionals in this sector
* Ability to meet the unique needs of service users such as gender or cultural considerations
* Possession of a full driving licence and use of a car, or otherwise able to meet travel requirements


Responsibilities

* Ensure adherence to all contractual targets and KPIs, reporting to the Short-term Intervention Service Manager as required
* Provide practical support and ensure recording of service outcomes wherever appropriate
* Sit with service users to keep them safe and improve wellbeing whilst clinical teams mobilise and begin clinical support
* Work in accordance with shift patterns
* Work closely with the Intensive Support Team (IST) and the Specialist Autism Team (SAT)
* Dedicate time to complete individual/team development and debrief, service development, administration and reporting, and attend required meetings

The Essentials

* Salary: £28,938 per annum
* Hours: 12 hours per day/night on 4 days on / 4 days off basis (Day shift – 8:30am – 8:30pm, Night shift – 8:30pm – 8:30am)
* Location: Hybrid Working – a few days per week from the Derby office, a few days from home depending on service needs and working at clients’ homes on a 121 basis
* Contract: fixed term (end date: funding dependent, to be discussed further at interview)
