Job Overview

We are seeking a Band 7 Palliative Care Clinical Nurse Specialist (CNS) in Carmarthenshire, primarily based in Llanelli. This is an excellent opportunity for an experienced specialist palliative care nurse to develop and enhance local services, delivering high-quality, patient-centred care for those with life-limiting conditions.

This role will mainly be community based, with some requirement to support secondary care for approximately one day per week

As a Band 7 - Clinical Nurse Specialist, you will provide expert clinical assessment, symptom management and psychological support. You'll act as a key contact for patients, carers and professionals, offering advice, education and coordinating care across settings.

You'll work closely with multidisciplinary teams to support seamless care pathways and promote best practice. The role includes delivering specialist training across primary and secondary care and contributing to audit and quality improvement initiatives.

Applicants must be NMC-registered with substantial specialist palliative care experience, advanced clinical knowledge and skills, strong leadership, advanced communications skills, emotional intelligence, resilience and the ability to work both independently and collaboratively are key.

Main duties of the job

The purpose of this role is to provide specialist nursing care, advice, guidance, and leadership, and to take responsibility for a specialist nursing service (as required).

You will assess, plan, implement, and evaluate evidence-based nursing care and provide expert advice and clinical practice.

You will be able to work autonomously and manage your own workload whilst leading and managing a team of specialist nurses, providing specialist education and training.

You will be expected to work collaboratively and co-operatively with clinical colleagues to develop services and the quality of care delivered, providing leadership in the development of clinical services within the specialist field.

You will at all times demonstrate the Health Board's values and ensure that others demonstrate the same values.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

Five community hospitals: Amman Valley and Llandovery hospitals in Carmarthenshire; Tregaron Hospital in Ceredigion; and Tenby and South Pembrokeshire hospitals in Pembrokeshire
Two integrated care centres: Aberaeron and Cardigan in Ceredigion, and several other community settings
47 general practices (six of which are health board managed practices); dental practices (including four orthodontic); 97 community pharmacies; 43 general ophthalmic practices; and 8 ophthalmic domiciliary providers
Numerous mental health and learning disabilities services
