Operating within a multidisciplinary environment to provide appropriate, professional Social Work support with a focus on meeting the social care outcomes as articulated in the Department of Health’s Adult Social Care Outcomes Framework. A focus on undertaking carrying out regent work including new adult assessments, safeguarding enquiries and reviews of care and support needs.
