North London, has a vacancy for a Mental Health Nurse (Care Co-ordinator). The core of this role is to support the delivery of care, which is currently in the form of New Models of Care in the North London NHS Foundation Trust’s HMP Pentonville.
Main duties of the job
* Work as a member of the Mental Health Service at HMP Pentonville, supporting the delivery of early days in custody, planned care, unscheduled care and release and transfer services.
* Provide sound clinical skills in mental health practice, including CPA care coordination, group work, risk assessment and management, and crisis intervention.
* Manage your own caseload as directed by the Mental Health Hub Manager and act as the CPA coordinator where appropriate, initiating planning and review of care plans under enhanced CPA.
* Conduct skilled assessments of patients’ emotional and psychological needs, based on complex data obtained from the patient and other sources.
* Deliver psychosocial interventions for patients with moderate to complex mental health difficulties.
* Facilitate group work jointly with multidisciplinary colleagues, including education staff.
* Complete clinical risk assessments at first and subsequent assessment interviews, accurately recording and reporting key risks.
* Develop care plans in collaboration with the patient, carer and other relevant parties, and review and evaluate treatment plans at agreed intervals.
* Act as lead professional for those on the allocated caseload who are not under CPA.

Staff may be required to work at different locations within Barnet, Camden, Enfield, Haringey, Islington or other places deemed reasonable by the Trust on a temporary or permanent basis.
Person Specification
* Dip/BSc (Hons) in Mental Health Nursing, Social Work or Occupational Therapy.
* Evidence of further post‑registration education and training or an equivalent teaching and assessing qualification.
* MSc or MA (Hons).
* Post‑registration experience as a Band 5 Social Worker or Mental Health Nurse (or Occupational Therapist) within a forensic setting.
* Recognised training in teaching, mentorship or supervision.
* Experience with neuro‑developmental disorders.
* Ability to undertake risk assessments using standardised or semi‑structured methods.
* Ability to care‑coordinate within CPA and collaborate with other agencies.
* Excellent communication skills, both oral and written, and the ability to work independently and autonomously.
* Specialist knowledge of the Mental Health Act 1983 Part II and Part III (as amended by the Mental Health Act 2007).
* Eligible for a Disclosure and Barring Service check under the Rehabilitation of Offenders Act (Exceptions Order) 1975.
Salary
£46,419 to £55,046 a year, Per annum including HCAS.
