Health Visitor

An exciting opportunity has arisen for a Part Time – 30 Hours Health Visitor to join the 0-25 Healthy Child service in Wirral, working for Wirral Community Health and Care NHS Foundation Trust.
The post requires strong communication, interpersonal and leadership skills, and the ability to be adaptable, reliable, compassionate, and approachable. Flexibility, solution‑focused thinking and support for service development are essential.
You will work as part of an integrated 0-19 Team and will support the full delivery of the Healthy Child Programme, including responsibility for supporting the universal and partnership plus families.
You will also be involved in a range of public health activities within a geographical area, working with individuals, groups and communities.
As a key stakeholder in the protection of children, you will collaborate with other agencies to meet their needs. You should have a good knowledge of Clinical Governance, Risk Management, Audits and relevant Professional Codes.
Main duties of the job
Improve health and well‑being by identifying health needs, raising awareness, and influencing the broader context to enable people to improve their own health. This is achieved through the health visiting contribution to public health, which takes account of the different dynamics and needs of individuals, families, groups and communities, focusing on health promotion and prevention of illness, ill‑health and injury. Work in partnership with the Local Authority and partner agencies, manage an active caseload, develop and deliver health care and health promotion using evidence‑based practice and professional leadership and management, promote positive health to individuals, families, groups, communities and relevant agencies, contribute to the development of best practice, facilitate shared learning to develop services and initiatives that meet local needs in relation to the aims and objectives of Wirral Community Health and Care NHS Foundation Trust, and develop effective links with key people in delivering services to enable people to improve their own health.
Qualifications & Training
* RN (Adult or Child/RSCN) or RM
* BSc/Public Health Nursing – Health Visiting or equivalent
* Motivational Interviewing trained
* Solihull Approach trained
* V100 Nurse Prescriber
* Promotional Interviewing trained
Experience & Knowledge
* Experience of managing a Health Visiting caseload
* Evidence of continuing professional development
* Experience of leading the assessment, planning and evaluation of care
* Experience of integrated working
* Ability to use email, internet and word processing software
* Sound knowledge of safeguarding issues
