Job Purpose
We are seeking a dedicated and compassionate Leaving Care Personal Advisor to join the Children's Social Care Service in Sefton. The successful candidate will play a pivotal role in safeguarding and promoting the welfare of young people who are preparing to leave, or have recently left, local authority care.

Key Responsibilities
* Provide advice, guidance, and practical support to care-experienced young people.
* Take part in the assessment, preparation, monitoring, and review of Pathway Plans, ensuring a person-centred and empowering approach.
* Coordinate and facilitate access to appropriate services and opportunities.
* Maintain accurate and up-to-date records on ICS and other electronic systems.
* Promote positive health, well-being, and confidence in young people, supporting them to reach their full potential.
* Identify, assess, and report risks in accordance with safeguarding legislation and local procedures.
* Work collaboratively across agencies as part of the wider corporate parenting team.
* Attend relevant meetings and represent the department where required.
* Contribute to service development through team participation and professional learning.

Requirements
* Experience working with young people, ideally within a social care, housing, or youth support setting.
* Strong understanding of safeguarding principles and legislation.
* Excellent communication and interpersonal skills, with the ability to build trust and rapport.
* Competent in maintaining case records and managing caseloads effectively.
* Eligible to work in the UK.
