Caremark Rotherham & Sheffield are seeking a dedicated and compassionate Care Coordinator to join our experienced management team. We proudly deliver high-quality support to children and adults living with complex needs in the community and are rated ‘Good’ by the CQC.
About the Role
As a Care Coordinator, you’ll play a key role in ensuring that our clients receive consistent, personalised high-quality care. This is a dynamic position requiring excellent organisation, problem-solving, and communication skills within a fast-paced environment.
Key Responsibilities
1. Effectively coordinate and maintain Support Workers’ rotas, ensuring continuity of care and appropriate client-staff matching.
2. Allocate fully trained staff in line with client-specific care requirements.
3. Respond efficiently to telephone and email enquiries.
4. Use care planning and rostering systems such as People Planner and Access Care Planning (ACP).
5. Liaise with clients, families, healthcare professionals, and colleagues to ensure seamless service delivery.
6. Respond to changes in care requirements or scheduling swiftly and professionally.
7. Provide regular support and supervision to the care team.
8. Participate in the on-call rota as part of a shared team responsibility.
9. Hold a valid UK driving license, access to a vehicle, and the ability to obtain business insurance.
Key Requirements
10. Previous experience in a care coordination or similar administrative role within health or social care.
11. Strong IT skills and proficiency in Outlook, Word, and Excel.
12. Ability to prioritise tasks and manage competing demands under pressure.
13. Excellent communication and interpersonal skills.
14. A strong commitment to person-centred care and high standards of service.
