Responsibilities
* Work alongside WMAS and other system partners to deliver joined‑up care.

* Adhere to BCHC policy and procedures.
* Ensure evidence‑based care is practiced and promoted using triage tools, exercising clinical judgement and expertise.
* Act as a first contact exercising a high degree of professional autonomy and critical judgement in providing complex clinical triage assessment for patients referred with acute and long‑term condition needs.
* Identify red flags and respond promptly, balancing patient safety, service guidelines, and patient preference.
* Initiate investigations, interpret diagnostic results, plan, manage, and evaluate care where appropriate, with support from Advanced Clinical Practitioners and Clinical Practitioners.
* Make direct referrals as appropriate for medical assessment and diagnostic procedures within agreed pathways.
* Signpost, refer and support access to health services to reduce inappropriate attendances and admissions to A&E.
* Promote participation by the team in quality initiatives such as falls prevention.
* Safeguard individuals from abuse by reporting incidents involving potential or actual abuse and acting upon concerns immediately.
* Promote health through collaborative working with patients and their relatives.
* Provide clinical expertise to reduce unnecessary hospital admissions and support admission avoidance, including effective history taking, clinical triage, and implementation of care services for acute and long‑term conditions.
* Partner with patients, carers, and other health and social care agencies to promote independence and develop individual care plans.
* Support other team members’ roles and contribute to future service development by providing professional advice and training.
* Rotate into clinical teams periodically to stay current with community services and maintain face‑to‑face practice.
* Work within a flexible working pattern over a 7‑day week.
