We are seeking a part‑time Health Care Assistant to join our Fenland District Nursing Community Team. In this role you will be trained and supervised by registered professionals and will follow a plan of care designed for each service user. This is an excellent opportunity for an experienced, motivated and enthusiastic individual to provide care in patients' own homes.

Cambridgeshire & Peterborough NHS Foundation Trust (CPFT) delivers integrated community and mental health, learning disability and social care services of the highest standard. Our clinical teams provide a wide range of NHS services, including adult and older people care, forensic and specialist mental health, learning disabilities, primary care and liaison psychiatry, substance misuse, social care, research and development.

To perform this role you must be able to travel independently around the county, hold a full UK driving licence, and have access to a vehicle. Public transport is not an acceptable alternative.

Applicants may be required to complete the new national Care Certificate within 12 weeks. The programme sets out 15 national care standards confirming that staff have the skills and knowledge needed to provide safe and high‑quality care.


Job Responsibilities

* Work as a member of an interdisciplinary community health care team, using skills from nursing, allied health professionals and medicines management to undertake tasks as delegated.
* Assist in the delivery of high standards of health care and support patients within their community setting by following prescribed packages of care.
* Apply competency‑based treatment techniques to patients with specific conditions under the supervision of a qualified practitioner, following a prescribed treatment plan.
* Deliver functional assessments, personal care, medication prompts, including mobility and exercise; personal and domestic ADLs; and equipment provision.
* Make judgements on patients' responses to prescribed treatment plans and report findings to a qualified professional in line with the competency framework.
* Overcome barriers to understanding, for example patients with hearing or cognitive impairment.
* Communicate with patients and carers, exchanging factual information, reassurance, tact and empathy.
* Establish and maintain appropriate working relationships with colleagues, patients, carers and other health care professionals.
* Ensure that equipment is used safely, is properly maintained, and that relevant training is followed.
* Work under guidance on specified tasks, taking direction from registered and non‑registered staff as appropriate, with regular clinical support and supervision by a qualified practitioner.
* Demonstrate an awareness and understanding of consent, and obtain consent as required following organisational policies.
* Attend relevant external and internal courses to extend knowledge or gain relevant skills to improve clinical practice as identified in the Personal Development Review.
* Adhere to universal precautions and infection control policies, given exposure to bodily fluids, infected material and blood products.
* Attend meetings as required.
* Ensure patients' personal care is provided and report any changes to professionals, especially pressure areas.
* Work within the wider disciplinary team and recognise own role within it.
* Promote patient independence and self‑management.
* Provide and maintain a safe, comfortable and dignified environment for all patients/clients and carers.
* Practice the principles and philosophy of individualised patient care in accordance with Trust policy, procedures and regulatory body guidelines.
* Maintain appropriate and timely documentation of each patient contact, ensuring these are recorded on SystmOne or RIO within 24 hours.
* Plan and prioritise own visits to delegated patients; ensure documentation on SystmOne.
* Ensure that any data taken or shared is undertaken with regard to Trust Information Governance and Information Security policies.
* Assist in the implementation of audit processes under direction from registered staff.


Qualifications and Experience Required

* Experience of working in the community.
* Experience of working within health or social care.
* Experience of caring for and supporting adults and older people.
* Demonstrated commitment to high‑quality care and service provision.
* Good communication skills.
* Ability to work flexibly and as part of a team, as well as independently.
* Current full UK driving licence and access to a vehicle.
* Ability to travel independently across the county.
