Responsibilities
* Conduct memory assessments and deliver diagnosis in collaboration with a multidisciplinary team, following NICE Guidelines and evidence-based practice.
* Carry out risk assessments and implement risk management plans in partnership with service users and carers.
* Safeguard patients and carers according to safeguarding policy and protocol.
* Coordinate care with partner agencies, making referrals tailored to individual needs.
* Act as duty nurse for screening referrals, providing advice and signposting.
* Supervise and support junior staff and trainees as delegated by the EMDASS Team Leader.
* Develop care plans that promote independent living in the community.
* Provide ongoing post‑diagnostic support, including titration reviews, and arrange appropriate follow‑up and discharge plans.
* Maintain high‑standard record‑keeping and documentation in accordance with Trust policies.
* Participate in audits, transformation projects, and propose improvements to pathways, practices, and procedures.
* Attend meetings, training, and statutory sessions (e.g., RESPECT, Basic Life Support) and drive to service users’ homes or other sites as required.
Required Qualifications
* Degree, diploma or equivalent in Nursing (Mental Health Nursing).
* Registration with the Nursing and Midwifery Council.
* Mentorship Preparation Course completed.
* At least two years post‑registration experience, including one year in Older People’s Mental Health Care.
* Post‑registration course in Dementia.
* Commitment to become a non‑medical prescriber.
* Understanding of end‑of‑life care in dementia.
* Knowledge of relevant legislation (Mental Health Act, Mental Capacity Act, National Service Framework, National Dementia Strategy, Care Programme Approach).
* Good knowledge of Microsoft Word, PowerPoint and Excel.
* Demonstrated ability to manage distressing and challenging situations.
* Evidence of expert clinical skills in dementia care.
* UK professional registration.
Desirable Criteria
* Pre‑ or post‑registration training or short courses in mental health and/or older people’s care.
* Community and inpatient nursing experience.
* Experience in supervision and leadership.
* Experience in research and audit.
